Case Illustration: Fibular & Periprosthetic Tibia Fracture

A 77-year-old female patient sustained a tibia and fibula fracture after a fall. Past medical history was significant for coronary artery disease and prior TKA with infection. The patient was treated operatively to increase fixation and provide a combined construct with the IlluminOss system and a plate to facilitate early weight bearing. After gaining a good reduction and traction, an incision was made at the medial malleolus. An awl was used to gain access to the medullary canal and reamed through the canal. An IlluminOss implant was inserted appropriately, filled with liquid monomer, and cured with visible light. Additional fixation was achieved by adding a second IlluminOss implant into the fibula through a small incision in the lateral malleolus. The implant was filled with liquid monomer and cured with visible light. Attention was then placed to supplement fracture fixation of the tibia with a plate and screw construct for support on the distal end in the periarticular region and periimplant screws proximally. The patient was instructed to weight bear as tolerated immediately after surgery. At 6-weeks follow up, the patient was healing well with visible callus formation on x-ray.
